  1. The caller (an adapter, the gateway or your code) sends the stage, the content and the context (tenant, app, agent, conversation).
  2. The API key is bound to the tenant; the rate limit, admission control and the licence pre-flight apply.
  3. The policy resolver merges the effective pack chain for (tenant, app, agent) and returns the stage’s rail graph.
  4. The language pass detects every sentence’s language; detector rails route segments to that language’s detector.
  5. Rails run level by level, in parallel and each with its deadline: detector calls or built-in checks.
  6. Findings are aggregated; every rail’s onFinding action (deny, redact, rewrite, hold, escalate, flag) folds into the decision; the strongest wins.
  7. The actions (redaction spans, a rewrite, a prepend) and the labels return with the answer; a hold opens an approval record.
  8. The event is written to the tenant’s chain; the caller receives a ledgerId. Async rails append a follow-up event later.

The nine stages

The caller says which stage it is at; the policy pack decides which rails run there. A stage is where the content sits in the agent loop; the same text passes different rails at input and at output.

StageWhat is checkedTypical rails
inputUser or caller text before the modelinjection.direct, jailbreak, pii, topic, secrets, budget
retrievalRAG chunks before they enter a promptinjection.indirect, pii, yara
tool_callTool name and arguments before executiontool.args, tool.args.injection, labels.sink, tool.authz, exfil
tool_resultTool output before it re-enters the modelinjection.indirect, pii, secrets, code.tool_result
outputThe complete model responsecontent.output, pii.redact, groundedness, system_prompt.leak, conversation, schema
output_chunkEvery sentence of a streamcontent.output, pii.redact, exfil
memory_writeWhat is about to be persisted for later turnsmemory.write, memory.authz, pii
fileBytes and metadata before storagefile.analyze, file.type, file.active_content, file.hidden_text, yara
agent_traceA whole trajectory, asynchronoustool.trajectory

Decisions, modes, postures

DecisionMeaning
allowNo findings; the content passes as is
allow_with_findingsFindings, no action; written to the ledger (the forced outcome of shadow mode)
redactContent returned with masks applied; actions[] carry the spans
rewriteContent replaced, for example a refusal template; prepend is a form of it (an AI notice before the answer)
denyContent is stopped; retracted on a stream
holdAn approval record opens; the caller pauses, the run resumes on approval
escalateAllowed now, an async judge queued; the result goes to the ledger and the webhook

Packs that require structured output also produce reask: when the answer does not meet the schema the caller asks the model again with the errors, and the pack’s fallback applies once the attempts are spent. A caller that does not re-ask treats reask as deny.

ModeBehaviour
enforceDefault; the decision is applied and a licence transaction counted
shadowFull evaluation, nothing blocked; the answer says in enforcedDecision what enforce would have decided; not metered
auditAsynchronous: 202 + ledgerId; checks the caller cannot wait for (agent_trace, post-hoc review)
offRail off, per pack and scope
The posture says what happens when a rail cannot run. Security rails ship closed: with no detector or past the deadline the check is denied. Quality rails ship open: the content passes and the skipped rail is written to the ledger. The pack changes this per rail; running a new rail in shadow first and closing it once measured is the recommended path.

Tool calls and approval

A tool call cannot be undone, so at the tool_call stage four deterministic rails decide together within milliseconds. The model judge runs only when asked and mostly asynchronously.

  1. tool.args: the arguments are validated against the per-tool JSON schema, size caps, SSRF classes and the egress allowlist.
  2. tool.args.injection: every string leaf is scanned by JSON path for SQL, command, path-traversal, RFI and XSS patterns (libinjection plus an OWASP CRS subset).
  3. labels.sink: the confidentiality and integrity labels travelling with the content are enforced at the tool’s sink class; untrusted content or PII cannot reach an egress tool.
  4. tool.authz: Cedar policies permit, forbid or hold over default deny; tenant → app → agent inheritance, the agent’s deployment state and the workload identity as attributes.
// Cedar: payments above the four-eyes limit are held, not denied
permit (principal in Tenant::"tnt_acme", action == Action::"call_tool",
        resource in ToolGroup::"payments")
  when { context.args.amount <= 1000 && context.args.currency == "TRY" };

@advice("hold:four-eyes")
permit (principal in Tenant::"tnt_acme", action == Action::"call_tool",
        resource in ToolGroup::"payments")
  when { context.args.amount > 1000 && context.labels.integrity == "trusted" };

A hold opens an approval record. The profile names the approvers, the four-eyes requirement, the SLA, the escalation target and what happens on expiry. The caller waits with a long poll or re-sends the request once approved; approval is given in the console, over a webhook or on Aginies’ approval card. Every approval is its own ledger event.

Information-flow labels and memory

Every piece of content carries labels: integrity (trusted/untrusted), confidentiality (public, internal, tenant, pii), PII categories and source. Tool output and RAG chunks are untrusted by default; a PII finding raises confidentiality; an indirect-injection finding forces integrity to untrusted. Labels only go up and return with every answer; the caller stores them with the value and sends them with the next call.

This solves the two-call case: a tool_result marks the content untrusted and PII, and the next tool_call to an egress sink is denied. The pack may declare the output of the organisation’s own systems (core banking, CRM reads) trusted; undeclared tool output is untrusted.

Memory writes are a stage of their own. memory.write requires provenance; untrusted content and instruction-like text cannot persist. memory.authz evaluates Cedar’s write_memory action by store, namespace and kind (session or long-term): untrusted content stays out of long-term memory, PII enters it only with approval.

Files, images and streaming

A file or attachment is analysed in an isolated child process before the rails run: the type is sniffed from the bytes, name/type mismatches, encrypted files, bombs and embedded files detected; text segments are extracted from PDF, Office, images and QR codes (OCR included). Active content such as macros, JavaScript, OLE and external templates, invisible text and hidden instructions are separate rails. Image moderation uses a multimodal guard on the GPU tiers.

On a stream the gateway or the SDK splits the model’s answer into sentences and checks each at output_chunk. A cleared sentence is released; one that must be masked arrives with its masked text; content that must be stopped sends a retraction frame and the stream ends. At the end the whole answer passes the output stage once more; that is the text of record.

When a detector is down

Nothing fails silently. An unreachable or broken detector records the rail as skipped_error, a deadline as timeout, a busy detector as shed, one that cannot serve as no_detector; the rail’s posture applies in every case and the ledger event carries the skipped rail with its reason.

  • A closed rail that cannot run denies the check; an open rail that is skipped lets the content through. Both appear under “rails skipped” in the evidence pack.
  • Under overload quality rails are shed first; at the hard cap the request gets a 503 and a shed ledger event is written.
  • The health page shows every router’s and detector’s state, why it is degraded and which rails would fail closed; activation reports the same gap in advance.
  • Prometheus alerts: a router down (critical), a detector unavailable, rail errors, deadline misses, shed requests, a broken ledger chain (critical).
  • On the SDK side an unreachable Thulpar is a decision too: the stage’s posture applies (input, tool_call and file closed; the rest open) and the breaker counts it.
